Scientists measured what is really in pregnant women’s bodies

Researchers from several major universities pulled together one of the largest pregnancy exposure studies to date. They followed more than 5,000 mother–child pairs whose babies were born between 2000 and 2021. The team tested maternal urine for 113 common chemicals found in household items, food, water, and air. On average, each woman had 45 different chemicals in her sample, and the highest sample contained 64. This was not about factory workers. These were everyday American women living normal lives.

The chemicals fell into familiar categories: phthalates and newer plasticizers from soft plastics and personal care items, polycyclic aromatic hydrocarbons from air pollution and combustion, and a mix of phenols and flame retardant chemicals from treated fabrics and consumer products. Many of these substances are already suspected of messing with hormones or fetal growth. This study did not chase rare industrial toxins. It mapped the normal chemical background of modern pregnancy.

Chemicals linked to earlier births and smaller babies

After measuring the chemicals, scientists compared exposure levels with two key outcomes: how many weeks the pregnancy lasted and how much the baby weighed at birth. Multiple phthalates and alternative plasticizers showed a consistent link with earlier delivery. Some specific compounds stood out. Higher levels of diisononyl phthalate breakdown products lined up with slightly shorter pregnancies and higher odds of preterm birth, while detection of phthalic acid was tied to a stronger shift toward earlier delivery and greater preterm risk.

Birth weight told a similar story. Phthalates, replacement plasticizers, and polycyclic aromatic hydrocarbons were all associated with lower birth weight for a baby’s gestational age. That pattern matches the risk profile for “small for gestational age” infants, who can face long-term health and developmental challenges. The effect sizes for each chemical were modest, but the direction was strikingly consistent across several chemical groups. That is what has environmental health researchers paying attention.

Regrettable substitutions and the myth of the “safe replacement”

One of the most unsettling findings is how often “replacement” chemicals showed up in pregnant women. Bisphenol A was removed from many products after public pressure, only to be replaced by similar chemicals like bisphenol S and bisphenol F. Studies now find bisphenol S in the vast majority of pregnant women tested, with levels tied to the same kind of concerns that got bisphenol A flagged in the first place. The new JAMA Network Open work extends that pattern to newer plasticizers meant to substitute for older phthalates.

This fits a familiar cycle in environmental health. Regulators and companies remove one problem chemical, then slide in a cousin molecule that behaves almost the same way in the body. It takes ten to twenty years for science to catch up and show that the “safer” substitute also disrupts hormones, lowers birth weight, or nudges pregnancy timing.

What a practical, family-first response looks like

This research does not mean every mother must fear her shampoo or sofa, and scientists are not calling for panic. But it does suggest a sensible set of priorities. First, expectant parents and doctors can focus on obvious sources: strong solvents, certain pesticides, and unnecessary fragranced or soft-plastic products, especially during the first trimester when fetal organs form. Second, regulators should stop rubber-stamping “replacement” chemicals without strong, pre-market safety data, especially when they are close cousins of known hormone disruptors.

Finally, the country needs long-term follow-up studies that track children exposed to these chemical mixtures and map real outcomes like learning problems, asthma, and chronic disease. That is slow, unglamorous work, but it answers a basic question: are our everyday products helping families thrive, or quietly taxing the health of the next generation? The new study on 45 chemicals in pregnant women does not close that case, but it makes clear that pretending everything is fine is no longer an honest option.
